What to bring to your first appointment
You can download our registration and medical history forms to save time at your first appointment. Alternatively you can fill out the forms at our office. You will need your medical information including your family doctor’s name, history of medications, illnesses and medical procedures, information on allergies, and your health insurance card and booklet.
Financial policy
Payment is due in full at the time of treatment. If you don’t have dental insurance or if you need assistance paying for your treatment we participate in CareCredit for dental financing.
Insurance
We participate in Delta Dental, Blue Cross and Blue Shield, and Met Life insurance plans, along with other dental plans. Ask our front desk team for more details.
